Transplant renal artery kinking: a rare cause of early graft dysfunction.

چکیده
A 50-year-old man with end-stage renal failure due to adult polycystic kidney disease underwent a cadaveric renal transplant. The renal transplant was uneventful and the right kidney was transplanted into the right iliac fossa. There was a single artery, vein and ureter. The kidney reperfused well but there was no primary function. In the first 24 h he only made 69ml of urine and the serum creatinine climbed from 616 to 757mmol/l. A Doppler ultrasound of the transplant kidney was undertaken and this revealed a tardus and parvus abnormality of prolonged acceleration time, diminished acceleration index and loss of the normal early systolic compliance peak/reflective wave complex [1]. A repeat ultrasound later showed the same abnormality with an increased velocity of the main transplant renal artery indicating a high-grade renal artery stenosis. An angiography demonstrated two kinks in the renal artery (Figure 1). The kinks were treated by two stents, 6mm in diameter and 1.7 cm in length (Figure 2). The patient was heparinized for the next 24 h and then started on clopidogrel. Transplant renal artery kinking is a rare cause of early graft dysfunction [2]. This is usually due to

The goals of post-transplant management are to manage and treat postoperative complications, and develop a balanced long-term immunotherapy regimen that minimizes infection and side effects but controls rejection. While modern immunosuppressant regimes have reduced rates of graft loss due to rejection, they impart major risks for infection, growth failure, metabolic complications, and malignanc...
متن کاملEarly dysfunction of transplanted kidney revealed the cause of recurrent nephrolithiasis: a case report of primary hyperoxaluria
Primary hyperoxaluria (PH) disorder causes end-stage renal disease (ESRD). Missed diagnosis or relapse in transplanted kidney is common. We present a 36-year-old patient with a history of recurrent renal calculus which progressed to end-stage renal disease. He underwent kidney-transplant surgery. Renal function tests had worsening progressively at first-week post-transplant. Transplanted kidney...

There may be many reasons for the malfunctioning of transplanted organs. Percutaneous biopsy can easily be undertaken to give greater specificity to possible immunological causes of rejection. Acute venous or arterial thrombosis frequently leads to graft loss after a short period of anuria which justifies a diagnosis of vascular occlusion. This causes graft dysfunction or loss in 3.5% to 12.5% ...
متن کامل"The seagull cry": a sign of emergency after renal transplantation?
Duplex ultrasound is widely used to detect vascular dysfunction after kidney transplantation. We describe musical murmurs found in renal allografts. A 29-year-old man with an increase in serum creatinine 1 month after cadaveric kidney transplantation was referred for duplex ultrasound to exclude perfusion dysfunction. Renal arterial and venous circulation was normal. However, during Doppler spe...
متن کاملIncreased Expression of Toll-Like Receptors 2 and 4 in Renal Transplant Recipients that Develop Allograft Dysfunction: A Cohort Study
Background: The incidence of ischemic reperfusion injury (IRI) in early phase post-transplantation and activation of toll-like receptor (TLR-2) and TLR-4 remarkably impact the outcome of a renal allograft. Objective: To investigate whether the expression of TLRs in peripheral blood mononuclear cells (PBMCs) can predict the clinical outcome of kidney allografts. Methods: We obtained blood sample...
